Our goal is to extend the excellence of our extensive surgical practice to the facial esthetic and
hair removal needs of our patients. We will likewise provide therapy for pigmented lesions (i.e. old
age spots) and spider veins, Facial Rejuvenation is a physician run service to provide you the excellence
in care that you deserve.
Douglas W. Klotch MD, FACS member of AFPRS
Dr. Klotch is a clinical professor of surgery at USF. He is board certified in Otolaryngology and
Head and Neck Surgery with fellowship training in reconstructive and oncologic surgery. He has over a
25-year experience with facial reconstruction and LASER surgery. He is certified to use a variety of
surgical LASERS including those used for facial resurfacing, skin tightening, pigmented lesion removal,
spider vein removal and LASER hair removal. He is also certified for the use of Botox and injectable
fillers (Restylane and Juvederm).
Melissa Carmichael ARNP
Melissa is a practicing nurse clinician in my busy surgical practice. She has expertise in patient care
and surgery. She has also completed training in the use of injectable fillers and Botox.
Melissa adds an extra dimension with her expertise in facial care products. She is trained to
provide our patients with the ultimate in skin care products to enhance the expected results achieved
with LASER skin procedures.
